DESCRIPTION:
Family Home Care, the premier Home Health and Hospice Agency in Spokane County, has an opening on our team for a Part-time Home Care Coordinator to work and manage referral relationships across Spokane.
Responsibilities include working with a small team of Referral Relations specialists to coordinate patient referrals, determine appropriateness of home or Hospice care, and determine potential needs of the patients. Work with physicians, families, case managers, social workers, hospitals and other health care agencies in establishing an initial care plan for patients. Conducts visits to referral sources to form, maintain, and enrich the relationship of the referral sources to Family Home Care. Provide follow up, customer service, and education regarding the various services that Family Home Care provides.
Coordinates information from patients including demographic information, insurance information, and obtains all required paperwork.
Works closely with reimbursement sources and provide community education and outreach. May be assigned to make home visits. Other duties as assigned.
The anticipated schedule for this position is Friday through Monday with approximately 24 hours per week.
REQUIREMENTS:
EDUCATION: Bachelor’s degree in marketing is preferred
LICENSE: Clinical license is not required, however, a clinician (RN, RD, MSW) with marketing experience may be considered.
EXPERIENCE: Marketing experience in the Health Care Industry is preferred, however, proven experience in sales and marketing in any industry may be considered. New graduates may be considered with right demeanor.
OTHER: Must possess or be able to acquire (FHC can assist) a current CPR card (BLS for infant, child, adult), a TB (tuberculosis) test within the last 12 months (FHC can assist).
ATTRIBUTES: Must have the demeanor of a sales person. Must be able to develop appropriate and compassionate relationships with physicians, referral source staff and patients of various backgrounds and conditions, along with members of their family. Must be able to work on a team that may span different days/times using good communication and complete documentation. Must have excellent communication and prioritization skills. Must be able to multi-task and work effectively in a fast paced high energy environment. Must be able to work in various home-based conditions including pet dander, smoke, and various temperature environments. Must be able to lift up to 50 lbs and work safely to prevent strains or injuries to either yourself or others.
TRANSPORTATION & BACKGROUND CHECK: Must possess a valid driver’s license, reliable transportation, and proof of ongoing auto insurance is required. Must be able to pass a Motor Vehicle Driver’s check with ongoing maintenance of a good-driver’s history (we check once a year). Must be able to pass a complete background check which includes: Criminal History, EPLS exclusion, APS exclusion, OIG exclusion, Sex-offender registry, and a credit check (does not check FICA score) to verify financial responsibility to handle FHC money.
COMPENSATION:
