Summary: The Case Manager for Family Promise of North Idaho is responsible for providing the direction and tools for homeless families who are being sheltered in our program to regain self-sufficiency. Some travel and transportation is required. This position is under the direction of the Executive Director. Family Promise is a volunteer intensive program; the candidate for this job needs to have the ability to work with a wide variety of personalities.
Program Duties:
1. Provide case management, advocacy, and referrals to our guests
2. Assist with crisis management and conflict resolution
3. Issue written warnings as relevant
4. Schedule weekly face to face sessions with guest families
5. Provide intakes for program
6. Follow up with graduate participants
7. Complete required statistics, reporting and charting in a timely manner
8 Open and written communication with Director regarding guest service plans, needs and schedules, as well as guest successes.
9 Attend relevant meetings: staff, house and coordinator’s meetings
10 Collaborate with staff and volunteers
11 Perform related functions necessary to support the mission and core values of Family Promise
Minimum Requirements:
Bachelors Degree in Social Services; LSW preferred
Excellent interpersonal skills including the ability to work independently with little supervision and as part of a multi-disciplined service delivery team.
Demonstrated familiarity with community agencies and ability to access services.
MS Office experience including Word, Excel and Outlook
Valid Drivers license
Must be able to pass a criminal background check
